Insurance Information
As a courtesy to our patients, we accept assignment on many dental insurance plans. In order to file your claim, we need your member ID and verification of coverage. At each visit, we will confirm your insurance coverage. It is important that you notify us BEFORE your appointment of any insurance changes so that we may check on your eligibility prior to your appointment.

Please be aware that your insurance may only partially cover services and that coverage varies among insurance companies and even among employers. At each visit, we will confirm your insurance coverage and request payment for deductibles and/or copayments as necessary.
Payment Policy
Payment is due when services are rendered. We accept cash, checks, and all major credit cards (Visa, Master Card, American Express and Discover). Please call prior to appointment with any questions regarding co-pay amounts.
Cancellation Policy
If you are unable to keep your appointment we ask that you give our office at least 48 hours notice. There will be charges for multiple missed/last minute cancellations.
