Office Policies and Financial Information
As a courtesy to our patients, we accept assignment on many dental insurance plans. In order to file your claim, we need your member ID and verification of coverage.
Please be aware that your insurance may only partially cover services and that coverage varies among insurance companies and even among employers. At each visit, we will confirm your insurance coverage and request payment for deductibles and/or copayments as necessary.
Payment is due when services are rendered. We accept cash, checks, and credit cards (Visa and MasterCard). Please call prior to appointment with any questions regarding co-pay amounts.
If you are unable to keep your appointment we ask that you give our office at least 48 hours notice. There will be charges for multiple missed/last minute cancellations.
